To prevent infections keep it dry by placing a waterproof ear plug or place cotton in the ear and seal the outside with vaseline. Long term a tympanoplasty which closes the hole may reduce the issues and improve healing.

Pain, stress: That is fast but well below the maximal heart rate a 31 year-old woman will normally have during exercise.  Similarly, it's normal for significant pain or stress to cause a spike in blood pressure and heart rate (you may be familiar with the fight or flight response), and as long as it return to normal when at rest, then this is a perfectly natural and healthy response.

Infection: Most likely the ear drum perforated, water with bacteria entered into the middle ear, and now there is an infection.  Recommend antibiotic drops such as Floxin (ofloxacin) or ciprodex, keep the ear dry and don't blow nose.  Most perforations close but would see ENT if there is still drainage or hearing loss after a month.

Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex. 
Get HPV vaccine, if you are under 27 years of age.

Yes but depends : It depends on what you mean by hearing loss. A mild hearing loss may be permanent. But, you are correct that there should be no severe permanent hearing loss unless there is more damage than just eardrum rupture. It also depends on the context of the damage and healing and repair of the perforation. Your ENT doctor can give you more specific recommendations
