Arm Alarm: Your discomfort suggests compression of either blood flow to the arm or directly on a nerve.  In the case of blood flow the tingling occurs as the tissues are deprived of oxygen which irritate the nerve.  Direct compression of the nerve causes a similar irritation.  A sleep splint with a ball attached to a velcro waist belt can be worn to keep you from rolling onto that side & compressing the arm.

my left and right arms have the same diastolic bp, but systolic bp in right arm always ~15mmhg higher than left. when under stress (e.g. pulse 135bpm), the difference between r/l systolic bp increases to 27mmhg. why? is it significant?

: BP is often higher on the right arm but it's usually less than 10 mmHg. If your left arm radial pulse is as strong as the right, I do not concern. If the left radial pulse is weak, left subclavian artery stenosis can be the culprit but it is rare at your age unless you had procedure done in that area before. A bruit can be heard if the artery is stenosed. R arm BP should be used for assessing HTN.

Might be neck relate: Many possibilties here. Most likely cervical spine disease with possible nerve impingement causing the numbness. Other possibilities include thyroid disease, possible polymyalgia rheumatica.
Need to see your physician and be evaluated.

Question?: Not sure what you are asking. 
With such frequent symptoms, you should probably consider an ep study where they could attempt to ablate the accessory pathway and cure the problem. Otherwise, more aggressive medical therapy could probably get better control.

Nerve pinching: Could possess a nerve compression at wrist, carpal tunnel syndrome, but also possible for more proximal nerve compressions such as at elbow, cubital tunnel syndrome.  Have your doctor assess, as this is potentially readily reversible.

8 months post thyroidectomy, how likely will my left cord become un-paralyzed and accessory nerve healed. my right arms affected by trap nerve damage?

Up to 1 year: Most ENT surgeons will wait 1 year before they consider intervention to do a procedure on the vocal cord to improve your voice.  SO possibly 1 year before you give up on the possibility of recovery.

Neuroleptic drugs: Neurontin, lyrica, (pregabalin) or some of the other neuroleptics work well for this sort of pain.

what does it mean if you have no reflexes in left or right arms? endo doc hit both arms and no reaction. but both legs reacted very quickly

Nothing: If you are not having any problems and this was routine exam, as long as both arms reacted the same it means nothing. Often the reaction in the arms is very subtle and while it can be felt by the examiner it will not be as visible as the reaction in the knees.
