what is the definition or description of: infliximab allergy?

Infliximab allergy: In the true since of the word, it is better to say infliximab reaction. The "allergic type" reactions can occur because a large amount of protein is being given IV in a short period of time. Patients may experience shortness of breath, itching, wheezing, and a rash.  In rare cases patients may experience a decrease in blood pressure.  Most rheumatologist give medications before given the infusion to help mitigate the side effects.

Why do you need  it: This really depends on the  medical condition being treated. Remicade (infliximab) may have no symptoms. It can have side  effects like infection and if you are on it  you need careful monitoring.

Accepted Answer

Yes: Humira, enbrel, simponi (golimumab) have all been approved for the treatment of ankylosing spondylitis. It is very common for a patient to fail one medication and do well on another. However, i would give any medication a minimum three month trial before declaring it a failure.

POSSIBLY YES: Centocor which makes Remicade (infliximab) does have an assistance program. Please see the Remicade (infliximab) website. Also, there are organizations such as patient advocate association which offer grants to patients that are in need of assitance. Talk with your rheumatologist's office.

i missed a dose of remicade, (infliximab) scared that i'll have a reaction at next infusion. my infusions are scheduled for every 8 weeks, now its been 15 weeks.

Reaction unlikely: A reaction is unlikely but you can always get a dose of IV Benadryl (diphenhydramine) by the infusion nurse prior to the infusion.

how important is a colonoscopy within 5 years of previous one...i'm a crohn's patient on remicade (infliximab) and i'm not flared up. seems unnecessary and expensiv?

Very necessary: It is essential to screen for disease activity and also to screen for early intestinal cancers which are more common in crohn's.

is there a difference between remicade and simponi (golimumab)?

Ask rheumatologist: A patient can ask her rheumatologist about how the doctor chooses one over the other. They are the same type of medication, with simponi (golimumab) the newer one. They should have the same effects in patients. There may be differences in cost, availability, and insurance coverage. The rheumatologist would know more about possible side effects for her individual patient.

Maybe: If it is very minor, yes. If it is more severe and if you have a fever, then no.

Yes: The usual top dose is 10 mg/kg. however, that dose is not all that high if it provides good disease control.

is there a diet that can eliminate the need for humira/remicade in crohn's disease patients?

Accepted Answer

NO: If you have active crohns, liquid or a bland diet may only be slightly helpful. With active crohns resting the bowel or nothing by mouth may be the most helpful plan. Fortunately, there are multiple therapies to help active crohns. Please talk with your GI md or your rheumatologist.
