Musculoskeletal pain: Coughing can cause musculoskeletal pain, including what you describe. Also, if the cough is due to a viral infection, musculoskeletal aches and pains can be part of the viral infection. There are many causes of cough including acute bronchitis, bronchospasm, gerd, post-nasal drip or a combination of factors. If your cough persists, see a doctor for diagnosis/treatment.

NSAIDs/exercises: Antiinflammatories and home exercises usually work if caused by overuse or a mild injury. If pain persists or worsens you should see a doctor.

Rest: I would first try a period of rest and avoid aggravation.  Would advise starting a course of antiinflammatories and icing as needed.  Can typically start with 800mg of Ibuprofen per day or 500mg of alleve twice a day which are rx strength.  Gentle stretching also helpful and can google codman's exercises.  Also work on rows and scapular strengthening.  If not better in 3-4 wks seek attention.

Not Usually: Shoulder pain can be referred to the upper arm but not usually the rib cage.

Shoulder pain: There are a number of possibilities.  The most benign is a strain, others include rotator cuff issues such as a small or large tear.  Please see your doctor.  Peace and good health.

Hospital: Please go to the hospital. First, you don't know for sure that you don't have a fracture - even a minor fracture, particularly in the neck. Second, you may have hit your head or just the impact could cause a concussion. The concern there is for brain swelling or bleeding.
