It Depends: On what is causing the inflammation in the knee. Common causes include trauma, infection, inflammatory arthritis (gout, pseudogout, ra), meniscal tears, arthritis, and other ligament/tendon tears. A thorough physical examination by your internist or orthopaedic surgeon can help narrow down the differential.

Infection per patell: The first thing to clarify is this an infection in the per patellar bursa on the top of the kneecap. That needs to carefully evaluated immediately after than then we consider other causes of inflammation. This could be crystals 
like gout or pseudogout and then other things.
