Possible: Insect bites are possible but need to make sure it is not ringworm. Your description fits ringworm well. It is a highly contagious fungal infection of the skin and scalp spread by contact. Can be itchy and get crusty. It is treated with topical anti fungals like Lotrimin, Miconazole, or several others. In severe forms or locations not suitable for topicals you may need oral antifungals. Send pic

May be insect bites: You may have bed bugs, in which you may have to be treated appropriately. Allergy is a possibility as well as some other diseases. However, if you have no other symptoms, the first diagnosis to exclude would be bed bugs.

Many possibilities: How long this has been happening?   If Benadryl (diphenhydramine) and Hydrocortisone have not helped, it is time to consult a dermatologist if the problem has been going on for more than 2 weeks or becoming intolerable.

Hives ?: Two months of a rash, which looks like hives to you needs to be evaluated. Hives usually do itch as they are a histamine response and are not typically always in the same place.  This sounds more like flat warts.  In any case exam and evaluation by your primary physician or a Dermatologist is needed.

Nerve: Check for nerve compression including the wrist and elbow areas.  Consult an orthopedist and get some nerve conduction study done.  This is not my specialty and hope others in HT will comment on this as well.

Depends on what you: Are referring to. Chiggers can. Chigger bites often have a small red in the center of a welt  ; occur in areas where skin is tender like the  crotch, armpits, beltline, backs of knees, ankles. They are extremely itchy.
