Trauma.: Anal fissures are usually the result of trauma to the inner mucosal lining of the anus. Most commonly this occurs when the anal canal stretches after a hard, dry bowel movement. Other causes of a fissure include diarrhea or inflammatory conditions of the anal area. Less commonly, an anal fissure may result from foreign body insertion or anal intercourse.

Before treatment...: Could you be referring to fissures, fistula, abscess? Sexually transmitted disease (syphilis, gc, hiv, etc.) should be considered. Hemorrhoids & rectal prolapse can appear as an outward bulging. Anal trauma of any kind may cause mechanical injury. Skin eruptions need to be considered. Evaluation is recommended to distinguish serious versus less concerning reasons for your problem before treatment.

: Pilex is promoted as helpful in the treatment of anal fissure. However this product is not investigated by FDA type government agencies. I don't see any harm in trying the ointment, but discontinue it if you note any sign of worsening inflammation or discomfort.  I feel a proctologist does the best job of treating and following up of anal fissures.

No: Anal fissures are common, but not normal; tears in the lining of the anus; often assoc with hard stool, and/or tight anal sphincter.  If they are in the north/south locations of the anus can be seen in otherwise healthy people; other locations can be assoc with things like Crohn's disease; Crohn's fissures can be north/south too.
See your doc, gi, or colorectal specialist; may (not) be a big thing.

Fiber/hot baths: The hot bath will help relieve the spasms that are often present with a fissure.  Increase your fiber and water intake to keep the stool soft.  If you are not getting better within a few days see a colon and rectal surgeon who can often treat it without surgery.

See a Surgeon: Non-surgical treatment includes warm water soaks 2- 3 times a day, increasing fiber intake, drink lots of water, and taking stool softeners or laxatives. Witch hazel (tucks (witch hazel) pads) can help. Some relief in a day or two usually. It may take several weeks for the fissure to heal completely. Sometimes fissures heal without treatment. May require surgery.

May need surgery: You should be seen by a colorectal surgeon. This could be a sign of something else. If itbis chronic fissure, myou may need a sphincterotomy, a surgery where a short segment of the sphincter muscle is cut.

Unlikely: If it is a true fissure ,it is unlikely to heal in a day. More likely many days when there is no further trauma and it is given proper care.
