Geographic tongue?: Obviously without actually being able to examine your child, we can't know for sure what it is.  How long has it been there? Does if vary from day to day and week to week?  Any other symptoms?  Any pain or discomfort?  There are many possibilities, one of them is called geographic tongue which is benign and does not require treatment.  Consider a consult with a pedodontist or oral surgeon.

Wait two weeks: A general rule of thumb is to have any lesion in the mouth that persists two weeks or more evaluated by your dentist. Most lesions like you describe are benign but may still need removal if they persist.

Perhaps: Although I've never heard of this, anything's possible.  Raw garlic held in the mouth does not impart any health benefit, and will just give you horrible garlic breath.  If sore does not fully heal in 10 days to 2 weeks do not ignore it, see your Dentist.

See your MD: The symptoms that you describe should be diagnosed thoroughly by an md who has familiarity with oral lesions and concerns.  It sounds as if you are seeing an md related to your med history, but you may be having an adverse reaction to one of those or there may be a totally different explanation for the findings that you are describing.  Do not delay in addressing them right away!

Well, kinda...: I'm not quite sure what you mean by "spots, " so i can't say for sure. However, the very back of the tongue has all kinds of normal bumps and lumps all over it. It can also exhibit various color variations. And, it can change over time. If you have doubts about what you're looking at, just get it checked by a dentist or an ent.

Spots: What kind of spots? Could be taste buds. Are they red or are they white and cotton ball like? Do they rub off with a cotton swab? You should see your dentist or ENT for a diagnosis.
