Folliculitis: Folliculitis is an inflammatory condition affecting the hair follicles. It appears as a small localized area of pus surrounding the opening of a hair follicle. Older lesions that have lost the pus appear as red bumps surrounding the opening of the follicle. Treatment is usually with oral antibiotics.

Doctor can evaluate: Bumps may be conditions such as allergic reactions, eczema, comedones, folliculitis, etc... Each would have a different treatment, so a primary care doctor or a dermatologist can evaluate and give some recommendations. Rashes are hard to describe in words. Little white heads are more likely folliculitis. If quite itchy, an allergic reaction is more likely. A video consultation may suffice.

Rash: These could be of viral(herpes) origin). If new lesions appear only on one side of the back, get checked for shingles. In any event, a physician would have to inspect these eruptions in order to make a diagnosis. Treatment is available for viral(herpes) infections.

Perioral dermatitis: What you describe sounds like perioral dermatitis, a relative of rosacea.  Foods generally don't make it worse but cortisone creams and heavy moisturizers do.  We use anti inflammatory medications such as topical creams such as metronidazole, sulfur/sulfacetamide and oral antibiotics to reduce the inflammation.  It may take several months to get the rash under control.

ive had this rash off and on my face for about a year. it doesnt cover my whole face, just in an area or two. looks a little scaly and bumpy, sometimes with white heads underneath. its not painful or itchy. i thought maybe a fungal infection?help?

Many possibilites: There are many types and causes of skin rashes that could cause itching.  Several types of inflammatory conditions of the skin, allergic reactions, and sometimes bacterial, viral or fungal infections can cause rashes on the skin. Insect bites can also be the cause of a rash on the skin.  Have it evaluated by a professional and get the appropriate treatment.

Hot tub folliculitis: Healthy skin is resistant to infection, but when swimming, friction of bathing suit and moisture allow bacteria to infect hair follicles and cause the rash.   More common from soaking in a hot tub and the bacteria is usually pseudomonas. The rash usually clears without Rx if you keep the skin cool and dry.
