if one sided you could be having shingles ,that could be very painful.
if the rash both side ,definitely is not shingles. could be heat rash ,or eczema, or dermatitis 
apply lotrisom cream but that need a prescription
or you can use hydrocortisone cream from over the counter.
the best advise see a physician, in case you need a prescription medicine .

be careful, you might be very very contagious. There are some good antiviral medications. You should get checked out!

Is required to give you the most complete answer, but the possibilities include muscle strain/spasm (levator scapula, rhomboid, trapezius), referred pain from the spine (disc herniation, ligament sprain, osteoarthritis), among others. If persistent and not improving with 1-2 weeks of relative rest, over-the-counter analgesics and ice or heat, then see your physician for an evaluation.

Upper back pain: The pain that you describe is typical of an inflamed rib head as it inserts into the spine. Anti-inflammatory medications, stretching and massage can help. Apply ice at night and moist warm heat in the morning. Chiropractors are helpful with this problem. Injection of the rib head with a cortisone shot is another treatment option. If the pain persists then a thoracic MRI scan is warranted.

Severe back pain: Anytime i hear severe, i would suggest a physician evaluate. Regarding the location, pain upper left (near shoulder blade) can suggest muscle spasm/tendinitis of shoulder, thoracic spine/disk problem, lung/heart disease, stomach/pancreas/spenic disorder. Seeing an md could help to determine the actual location and determine appropriate testing to get a diagnosis. Too little information.
