If they are little : Bumps on the dark area around the nipple (areola) and white material can be squeezed out - it is likely montgomery glands. They secrete lubricant. If not this, please seek medical evaluation.

Folliculitis or...: It could be an infected hair follicle or it could be a little sebaceous cyst.  Assuming that it's small (1-2mm)  If it's bigger than 2mm, then you should see a doctor.  Any breast mass needs to be examined by a physician, unless it's solely a skin issue.

Doctor can evaluate: A doctor will have to get more of the story about the nipples, and do an exam (one can start with her primary care doctor). Nipple rashes can be from friction (from shirts, sports equipment like swimming in an inner tube, etc...), eczema, an allergic reaction, etc...  Nipple discharges can be from hormonal imbalances, eczema, allergic reactions, infections, medications, etc...
