Go to ER for eval.: Infection rate from cat bites is high. You may need antibiotics.  If you haven't had a tetanus shot in the last 10 years or your don't know - then you will a need another tetanus shot. Get prompt care. This can quickly become a serious problem.

Yes: Depending on the cause, depth, size, and potential contamination of the wound, punctures injuries may be treated by local cleansing, removal of any contaminants, removal of damaged or crushed tissues.  Sometimes mds will leave the site open to prevent trapping bacteria or non-vital tissues.  Closure is sometimes possible.  Important to have a doctor evaluate and guide in all penetrating punctures.

It depends.: As long as you are otherwise healthy and have an up to date tetanus vaccination it is reasonable to watch the area.  Wash it daily with soap and water.  If the area developes increasing redness, pain, or drainage see a healthcare professional right away.  If this nail went thru the bottom of a shoe this increases risk of a certain type of infection that you would best be given antibiotic for now.

Depending on the circumstances, a culture may be taken and emperic antibiotics may be given pending the results of the culture. Also, we recommend that a patient with a puncture have up-to-date coverage with a tetanus shot.

Pad it with a protective dressing and keep it clean, dry and free from further pressure/trauma.  If it begins to become red, swollen or the pain worsens, see your doctor.  If you haven't had a tetanus vaccine in the past 5 years, you need one.

Do not seal: Puncture wounds need to drain as have high potential for infection. They should not be surgically closed. And should be irrigated daily and sometimes packed with a wick to avoid premature closure at the surface. If you see any signs of cellulitis or purulent drainage seek care with your pcp.
