: More info needed. I recommend consulting a specialist in internal medicine who will take a thorough history &amp; system review, perform a complete physical examination &amp; order appropriate testing in order to make a differential diagnosis &amp; advise you. You may be referred to other specialists like a cardiologist and/or neurologist. Deficiency of calcium&amp;magnesium can cause diffuse twitching

Concerning...: You don't give a past med history or age, in order to ans this question the most import things to know is do you have DM or a history of vascular disease. This could be a hematoma on the underside of the toe which in that area can be very painful since the trapped blood causes pressure buildup. More concerning is possib of gangrene, espec if you have DM. Need more info. Rec pod, wound or HT cons.

if i lay on my back and put my legs vertically in the air my pedal pulse becomes visible and i can feel it strongly in my right foot. the feeling and visible pulsing go away when i lower my legs. it doesn't happen in left foot. should i be concerned?

: Since this maneuver isn't one that most physicians use during a physical examination, I'm not sure what the significance of your observation is. Because of gravity I wouldn't expect the dorsalis pedis or posterior tibial pulses would be more forceful with the legs vertically elevated. If conventional examination of these pulses , with legs horizontal or dependent, show normal pulsations no problem

Evaluation ASAP: Even without pain this should be evaluated ASAP.  It is always a good idea when something supposedly stable suddenly changes.

The success rate is about 80%. What you will need is probably 2-3 treatments spaced out in 1 year.

Phlebitis: See doctor. Superficial phlebitis will resolve with Aspirin and heat. However a deeper phlebitis can lead to serious complications. Your primary can examine you and order a doppler exam to rule out deep thrombophlebitis.
