Remember: I've worked with you in regards to your stone questions but the hypercalciuria without evidence of hyperparathyroidism or chronic acidemia(as hasn't been found in your testing) suggests this is idiopathic hypercalciuria(it's rather common in calcium stone formers.

Depends: Patients with mildly symptomatic hypercalcemia;  calcium less than 12 can be monitored. Avoid activities that aggravate hypercalcemia like volume depletion, prolonged bed rest or inactivity and a high calcium diet (&gt;1000mg/day)Go back to the Doctor who did the workup and knows you and discuss all the options.

Correct D deficiency: Vitamin d deficiency reduces intestinal calcium absorption and stimulates parathyroid hormone secretion which maintains the blood calcium in the normal range until it is so severe that a low blood calcium develops.  Correction of d deficiency restores the parathyroid hormone and calcium to normal.

No Hyperpara: The normal PTH level indicates that you do not have hyperparathyroidism.  Calcium also needs to be corrected for your albumin.  Assuming that it is normal ; given that you have swollen lymphnodes, other causes of hypercalcemia are more concerning such as sarcoidosis or lymphoma.  Checking can help as1, 25 vit d should be high, (25) oh vit d normal to low. You should talk to your doctor about that.

Kidney Failure: Kidney failure is another cause of secondary hyperparathyroidism.  When kidneys are not working well, phosphorus levels go up, and calcium levels go down.  These lower calcium level stimulate the parathyroids to make more pth.  Again, even if the calcium levels are in the normal range, they may be low for that person, and cause the PTH to go up.

76 yrs, female, stage 4 ckd, pth 285, normal calcium, phospatase levels. is this close to kidney failure? what to do?

Accepted Answer

Getting close: Stage 4 CKD is considered severe kidney disease with stage 5 considered end stage and requiring dialysis. The best thing to do is to see a nephrologist regularly and take  medications they prescribe regularly. You should also avoid NSAIDs like motrin and aleve (naproxen). It would be good also to avoid salt and drink no alcohol.

i have low vitamin d and have tried supplementing but my levels wont rise. i have normal calcium and magnesium levels. my pth is normal. how can i still have normal calcium levels and normal pth levels if my vitamin d level is low?

: How low is low? What did you use to supplement? The Vitamins D level you tested is not the “active” form of vitamin d that maintains calcium levels. The body is very intelligent, and can maintain normalcy im even in the face of mild deficiency. Chat with a nutritional MD or nutritionist.

That's 1 possibility: Besides a parathyroid tumor, it could also be due to low vitamin d levels, or kidney failure.

This is often seen: in fair complected people and may be a pre-cursor to Vit D Deficiency....you need NUTRITIONAL COUNSELING!
