regression of ptb in both upper lobes.
pneumonitis considered, both lower lobes.'

was diagnosed of ptb infiltrates on both upper lungs, went under medication for 1yr last 2009 gotnnegative results?

Sounds like: you had infiltrates suggesting TB infection and treatment was carried out in 2009. You only need a course of treatment ONCE  Regression means IMPROVEMENT but some residual scarring may remain

: The top tissues of either lung is the typical location of pulmonary tuberculosis infection and any findings suggesting an active infection in that location suggest it as a possibility. However, other tests are required to confirm that possibility and another germ may be the cause.

Pulmonary Tbc: Pulmonary tuberculosis (if that is what you meant by this abbreviation) is one potential cause of "pneumonia".  Pneumonia, or pneumonitis is inflammation of the tissues of the lung, usually confined to the air spaces and the intraalveolar septi (alveolae and the walls between them), excluding the airways themselves (bronchi, bronchioles, etc.). Confirm diagnosis. : The first thing you would need to do is confirm the actual diagnosis. Pneumonia and tuberculosis have vastly different treatment regimens and long-term implications.

Clinical history: So it depends on prior exposures.  If you clinically have cough, fever, mucus production more likely an acute infection like pneumonia.  However in endemic area, tuberculosis until proven otherwise.  In younger person without other lung issues it would be rare to have fibrosis in upper lungs.  See specialist if this does not resolve with antibiotics and repeat xray (priors?) for help

Pulmonary TB: Pulmonary tuberculosis is TB in the lung, which could be inactive or active (the treatment is different for each condition). A person can have PTB and still get almost any other disease known to man. Having PTB does not prevent one from getting other diseases.

what is the treatment of pleurodiaphragmatic adhesion left? he was never diagnosed to pneumonia or ptb. 3 months ago, he had an xray and the result is negative but right now he undergo xray and the result is pleurodiapragmatic adhesion left.

Adhesion: Most adhesions are the harmless  remnants of a prior infection like bronchopneumonia.  Your physician should review the xray with the radiologist and ask for advice as to whether this finding is concerning, or whether additional studies like a chest ct scan, or referral to a pulmonologist would be advisable.
