Multiple: There are several criteria.  But basically would boil down to having failed legitimate non operative care and that your case would benefit from the release.  Therapy is the most common nonoperative treatment.  The surgery is typically done for a tight psoas often producing a "snapping hip syndrome".

Varies: You have a complicated case and as such there are many possibilities.  A repeat tear of the labrum and scar tissue would probably be among the most likely.  An exam and possibly an MRI/arthrogram would probably give some idea.  Don't hesitate to seek second opinion as well.

Scar tissue: Difficult case.  If you've already had surgery for scar tissue removal, then there's certainly a chance that you've formed more scar tissue again which is causing your recurrent symptoms.  Sometimes a steroid injection into the joint may help.  I would recommend discussed your problems with your surgeon.

i had a right hip joint replaced with a ceramic and titanium unit. the entry was anterior. i have severe groin pain. it's appears to be the psoas. the surgeon thinks there may be cup impingement, has indicated psoas release surgery. i'm in disbelief.

Get second opinion.: Anytime you are not comfortable with the information you receive from any doctor, get a second opinion.  It is the smart thing to do.  Though well-adjusted docs have no problems with you doing this, and often encourage you to do so, others may get peeved about it.  If so, it says something about that doc's attitude and/or professionalism.   You are always entitled to a second opinion.

I do not know: Perhaps a surgeon can weigh in here as well. If you have a psoas problem i would suggest seeing a chiropractor and a neuromuscular massage therapist. You may be able to get release/relief non surgically.

why does my thigh, front feel like it will give out, not support me, if i tighten the thigh muscle? 15 months post-op total hip replacement, anterior approach. scheduled for psoas tendon release for snapping hip syndrome. concerned about the release?

: Likely question to ask is whether the thigh problem is due to disuse atrophy or nerve problem associated with prior surgery.  An EMG study can help sort this out, and could be easily done prior to further surgery.

Infection elsewhere: Used to be TB was major factor. Now it is more from appendicitis, kidney infection, diverticulitis, spine infection.

No: This muscle lets you kick your leg up at the hip.  Everybody has one on each side. They're impressive on scans and someone may think they are a mass.

what is psoas muscle contracture?

Accepted Answer

Loss of hip motion: Inability to get the hip totally straight. The psoas muscle, which helps to flex the hip forward, may become shorted or contracted, either from a neurological condition or if someone has been sitting with the hip flexed (as with someone who spends a bunch of time in a wheelchair).
Therapy to stretch the hip can help but sometimes surgery to release or lengthen the tendon is required.

Stretching.NSAIDS: slow passive stretching and judicious use of Advil or Aleve (naproxen) if not allergic
