After long term use: Prozac's  half life is 4-6 days  so taking one daily is the 
convenient dose.

Timing not critical: Prozac (fluoxetine) is a long lasting medication and does not need to be split up. If you are sensitive to medications you can start with twice a day for a week or longer.  If no issues is is easier to dose the 40 mg  one time a day after that.

Yes: This can be a side effect of fluoxetine. It may be easily managed by taking it at night; or, you may wish to consider changing to a different medicine.

Prozac (fluoxetine): Please only increase or change medications under your doctor's direct supervision. I would take it which ever way your provider suggested.  It is usually safe to increase dose by 10 or by 20mg at a time.

Don't worry: Not at all. The max daily dose is 80mg/day. Also, a pill box may help make it easier to remember.

Once daily: You can take Prozac (fluoxetine) once daily, preferably in the morning. There is no benefit to dividing the dose. You should schedule a follow up with your doctor 1 month after starting the new dose, to determine when and if you need to increase the dose. Good luck.

Won't fix depression: Antidepressants all need to be taken on a regular daily basis to work as antidepressants.  The usual daily dose of Prozac (fluoxetine) is 20 mg, but some people need more.  Prozac is sometimes prescribed instead for pms symptoms, in which case it need not be taken continuously.  The dosing is best discussed in person with the prescribing doctor.
