50 year old. psa rose from 1.4 in september to 3.9 in march. been have off and on dull muscle aches in hip and back. no other symptoms. is this for sure mean i have stage 4 prostate cancer? or can the hip/back aches be also symptom of stage 1 or 2?

Age related: With a high p OM yjr 50-60 years of age prognosis is guarded.  Over 70 a stage iii prostate  cancer can do well on a drug like lupron (leuprolide).

Yes: Even for advanced prostate cancer, there are hormonal manipulations we can do to control the cancer. If the cancer responds to the medications, life expextancy can be years.

Highly variable: This depends entirely on the stage at which teh diagnosis was made and what treatments are necessary. If diagnosed and treated early, an entirely normal lifespan and normal health can be expected. But highly advanced disease, e.g. metastatic to bones of course is very serious, perhaps fatal within weeks or months. The patient's doctor is the one to ask!

Depends : Depends on the stage of the cancer and where it has spread ,it might be serious if spread to certain parts like liver,lungs,brain. Five year survival rate is over 90% but if the disease has metastatic un hopefully the rate drops into 30%

Prostate CA staging: Stage III means the cancer has spread out of the capsule or into the seminal vesicles, the glands that sit right on top of the prostate.  Treatment would consist of anti androgen therapy, a shot, and radiation.  There is always the chance the cancer could spread so a bone scan and CT is done to check and to also use as a baseline for the future.

Cancer rates: The chance of developing an invasive breast cancer during a woman's lifetime is 1 in 8.  The chance of developing prostate cancer is about 1 out of every 7 men.  About 80% of men over 80 years hold have prostate cancer.

More than 10 years.: The natural history of prostate cancer, unlike many cancers, is quite long & highly variable. A significant number of men will die of other causes before they die of prostate cancer. Recent randomized trials in prostate cancer (plco, erspc, pivot) have highlighted how difficult it is to determine survival in different subsets of men, and median followup in these trials ranged from 7-13 years.

6 years on average : Metastatic prostate cancer responds to hormone therapy for almost 18 months. Once  the cancer is hormone refractory the median survival is 6 years.

Serious: Metastatic prostate cancer is not curable with currently available treatments. Treatment, however, does improve quality of life and may add years to life. The first step is usually hormonal therapy. I recommend you consult a medical oncologist.
