None or: sciatica. Usually the back pain resolves with time. Sometimes there is muscle weakness or loss. May have numbness in the L5 or S1 nerve root distribution. With the diabetes, I would get an NCV-EMG to sort out what symptoms are due to neuropathy vs. radiculopathy.

Combination of issue: Some of the back issues are congenital, some due to injury or wear and tear.  None seems surgical, and your symptoms can respond to conservative therapy.

Avoid surgery: For axial neck and low back pain, and young age we rec avoiding surgery.  You need to first exhaust muscle rehab balancing the extensor and flexor muscles of both the low back and neck area. Over the counter nsaids are next.  If you have nerve pain in the arm or legs then you need to see a specialist and discuss the options.

Not usually. See MD: A cervical disc on the left can cause pain in to left shoulder/scapula and in to the arm. The pain can give a patient fatigue if sleep is impaired.  The heart should definitely be evaluated as soon as possible, though, as that could be potentially be life-threatening.

Evaluation: Surgeon should evaluate for symptoms warranting surgery (intractable leg pain, progressive loss of motor or sensory function in leg or inability to control bowel/bladder/ sexual function) and an MRI that confirms  a nerve lesion in a compatible location.  If indications are not present, medical management (injections, judicious rest, medications, physical therapy, and other therapies) is the norm.

my husband underwent a laminectomy & disc l5-s1 was removed. please suggest activities to strengthen his back.is swimming good for the back?

Accepted Answer

Swimming is great: Once his back skin is healed well (about 3-4 weeks) swimming can be done safely and is a great way to strengthen his core to prevent reinjury. Light yoga and plank exercises are also good way to strengthen the core.

Bulges: Discs naturally bulge as we age.  Unless there is evidence on MRI and on exam that the disc is actually pushing on some other structure (typically a nerve) then the disc bulge is highly unlikely to be the cause of back pain.  Look for other causes (muscles,  joints,  etc.) as for getting worse with menses,  has your doctor considered endometriosis?

herniated disc l5-s1. pain increased over a few months, improved but after starting gym again, became acute. i have been resting 2 months now and improving slowly. can walk 5 min before pain. should i just wait longer? is it likely to just go away ?

You have options: Would consider an evaluation and maybe lumbar epidural corticosteroid injection. Clarity will help

Accepted Answer

Yes and no: For the appropriately selected candidate who has also failed non-operative care, I do think a disc replacement can be a very viable option.  The profuse in general probably had more ongterm data to support its use at this point, which is not to say it is better( just more documentation that it is relatively safe and effective.
