These results discribed are disk disease/degenerative changes, there is nothing described to tell of how sever eit is so it is difficult to know what the extent is without seeing the images. The most important thing concerning the MRI is to make sure the finding are consistent with your symptoms in order to tell if it is of any concern. As we age, most of us have the abnormalities as described in your report, but the importance is deciding if this is the cause of your symptoms. This should be done by your doctor.

Call your doctor: Your doctor who has the complete report and who has  examined   you can give you more information. You have a herniated disc which may be putting some pressure on the nerve.   Your doctor can give you more information and recommendation for treatment.

Clinical correlation: The meaning stems from what your symptoms are and what your exam findings are and why you had the MRI in the first place.  If there is pain in that area, then THAT slipped disc pushing into the spinal cord could be the cause.  If the MRI was done for some other reason and there is no pain, that disc may not be meaningful.  It means there is material from the disc out of place however.

Strength training: Might help to see a physical therapist if an ortho has made DX of impingement. Can improve with cortisone injection, antiinflammatories, pt. Sometimes needs further work up and surgery.

what is the difference between an impingement and tear of a rotator cuff?

Severity of condtion: With impingment, the rotator cuff is being pinched, without necessarily being torn, between the acromion of the shoulder blade and the top of the humerus.   Raising your arm pinches the rotator cuff just like closing a door pinches your fingers if they are stuck between the door and door jam.  A rotator cuff tear implies that the tendon is not simply inflamed but is partially or totally disrupted.
