Surgery: Hurthle cell tumors are adenomas but has higher incidence of malignancy , if they are over 4 cm and in older population , surgical resection is needed , some even advocate total thyroidectomy, instead of lobectomy.

Since the pathologist read the specimen as undetermined, other areas of concern may include your thyroid gland. A biopsy may be discussed during your follow up visit to ensure that there is no further complication.

May be accurate: FNA is an aspiration of cells from a lesion or lymph node.  It may reveal cells characteristic of malignancy but does not show architectural structure to  help define the tumor.  Its like looking at bricks from a building and trying to tell what the name of the building is.  A true cut needle gives more information but a formal biopsy is the best way to make the dx of a lesion.

Get it removed: Any atypical cells in the fna biopsy raises the possibility of cancer in the thyroid gland. That means the need for surgical excision of the tumor/thyroid gland. You should seek an opinion from an endocrinologist asap.

Review with your MD: Usually, when one aspirates a thyroid cyst one may see "rare epithelioid giant cells".  Most simple cysts of the thyroid are benign. However, fine needle aspirate of cystic papillary thyroid carcinoma may also show this feature and may lack diagnostic cells. So, additional information is needed to interpret the cytology report.

However, hurtle cell adenoma is non-maligant.

Cancer: If there is evidence of cancer then total thyroidecctomy  is the recommended treatment. Thyroid replacement is very easy. Good Luck
