A pleural effusion would have been seen on the chest xray taken if it was significant.  If it has been a while since your chest xray and you have completed a course of treatment and you are still coughing or short of breath, then please see your doctor again for re-evaluation.

Yes, go to the DR.: Yes, that does not sound good, you need to be seen right away. You could have a pleural effusion, but that does not explain the nodules . I assume you had a chest x-ray? Someone sent you in for that , right?....Better go back and get fully evaluated.

Heart failure? : Do you have heart failure? There are several reasons for Pleural effusion. Depending upon how large you might need thoracocentesis or fluid drained and analyzed. If cough fever this could be infected or empyema thus requiring chest tube drainage. However it is usually only  on one side. In summary if this is a new Pleural effusion the cause must be worked up.
