Not that simple.: Generally expect to see a diuretic, beta blocker and angiotensin converting enzyme inhibitor in addition to meds to control underlying cardiac risk - meds for diabetes, hypertension, cholesterol, etc.  Many other meds may be added - hydralazine / nitrate, digoxin for example and others may be substituted.  Your cardiologist will know your case best.

Not really: Your numbers are within normal limit given your age in certain situation cholesterol build up starts in womb in patient with familial hyperlipidemia, usually  these patients have very high lipid levels. In an otherwise healthy person build up may begin in adolecent years as fatty streaks and the rate of build up depends on their diet, exercise, family history and the onset of disease e.g. Diabetes.

Safe weight loss: Most weight loss in the first days after treatment of congestive heart failure is water weight and may result in several pounds per day.  Once your doctor determines you are at normal body water, you can begin to lose fat weight, and most recommendations limit this weight loss to 1-2 pounds per week to avoid vitamin and mineral deficiencies.

Plaque : Please continue to see your doctor.  Ask about a natural product called Wobenzime.  It contains fermented soy which is used to soften the plaque for better blood flow. Yes: They can change in two years. The sad reality is that they can change overnight. Plaques rupture and that's a sudden event. We wish heart catheterizations could be crystal balls but, the reality is,  we still have no way to predict the future.  Your diabetes is a serious risk factor which, with rare exception,  you can't change.

Not by plaque clear.: Your heart is like an engine with 3 main parts: muscle (engine), blood supply via coronary arteries (pipes that bring fuel to the heart) and electric system that starts the engine up. Plaque affects the second - coronaries. Excercise is great for the first one - heart muscle itself.

Statins: Depends on lipid profile. Stains have varing effects on reduction of cholesterol profile. Pravastatin and Lovastatin - low side effects but weaker. Simvastatin - tend to avoid now given side effect issues. Lipitor (atorvastatin) good and now generic. Crestor brand only - good agent. Others available. Getting ldl, hdl, and triglycerides to goal is the key to preventing progression of coronary artery disease.
