Pituitary anatomy: Hi. The high resolution pituitary MRI has revolutionized pituitary endocrinology along with our highly reliable pituitary hormone assays. Pituitary MRI only gives info about anatomy. Using gadolinium contrast is very helpful, but still only gives anatomy info. We see pituitary tumors, infarct, etc. Or nothing. All imaging modalities have their limits of resolution. I hope your pituitary does well!

First confirm dx: Before imaging a dx of acromegaly should be confirmed with a growth hormone suppression test. Only after the biochemical diagnosis of the pituitary disease is made should imaging be done. There are other reasons for pituitary abnormalities, including nonfunctional pituitary adenomas. Iit would be a shame to have abnormal imaging and an extensive work up only to find that the dx is not correct!

is a pituitary mri different from an ms brain mri?

Merely technical: Looking directly at base of brain and pituitary requires slightly different cuts and angles, as the focus is directed to a small area of brain, but the software and pictures are handled in a similar fashion.  In ms, we tend to use specialized approaches, such as flair or double inversion recovery to see the white matter spots better.  Not needed for pituitary views, usually.

a brain mri found a 4x6cm lesion in the puitary gland described as a microadenoma. two weeks later a pituitary mri shows an 1.06x1.53 cm  area of heterogenous gland described as a evolving pituitary adenoma or apoplexy. what is an evolving adenoma?

Pituitary tumor: evolving meant changing/growing.  apoplexy is bleeding into tumor that can cause severe headache and even shock but can be more indolent.  Pituitary adenomas can aslo grow during pregnancy.

MRI: The levels sound more like hypothyroid.  I would have a prolactin level done first.  If you are having other symptoms i would follow with your physician.

can functional hypothalamic amenorrhoea cause long term imbalance? i had it for 3 years, a 2017 pituitary mri was normal. periods back but irregular with t4, tsh, estrogen, cortisol low normal & progesterone low. is it long term effect or a new issue?
