is there a sure-fire way to diagnose paget's disease of the breast? how long does the condition take to worsen? i have a itchy patch on my areola.

Biopsy: The only sure way to diagnose is by taking a biopsy and have a pathologist examine the tissue. It would be prudent to see a doctor as it is not feasible to give a meaningful opinion without examining you.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex. 
Get HPV vaccine.

Not in your age : It is rare in your age if no improvement get a second opinion crust or drainage or reaction to the nipple are a signs of Paget disease

See your MD: If you have any changes on skin of breast which does not go away. Or itchy skin ... it is always good reason to see your doctor. Page's disease of breast is very rare but very serious and unless proven otherwise - any skin itch or inflammation has to be suspected.  Please, see your MD ASAP for evaluation.

Pagets : dear lakeida,  Pagets is more often red, itchy rather than scaly.  Other possibilities include Tinea or other non-infectious dermatitis such as impetigo or piryriasiform dermatitis (especially with a scale).  The only way to truly know is a punch biopsy of the affected skin.

Possible: Most malignancies developing in the  breast begin within the lining cells of the milk ducts and invade as infiltrating duct cancer. The breast ducts form a confluence meeting in the center of the breast at the skin  level  as the nipple.  Those malignancies arising in the terminal milk ducts spill onto the areola as Pagets disease which is is a rare form of breast cancer in which cancer cells collect in or around the nipple. The nipple and areola often become scaly, red, itchy, and irritated.  When this area doesnt heal, biopsy is indicated.

Pagets: breast ?'s: Paget's disease of the breast is rare, but does originate in the nipple area, with a rash or scaling patch.  See a surgeon for a biopsy, as these kinds of symptoms are represented by simple chaffing of the areola with loose supporting garments, and most are completely benign.

It can be seen on x-rays, and can be a cause for bone pain.

Cannot estimate: No one even knows what percentage of older folks actually have some paget's lesions, since the disease is usually subclinical. If older folks with an unexplained elevation of alkaline phosphatase are assumed to have paget's, it's maybe 10%. The key is that paget's can be managed nowadays and of course it's largely hereditary. Let your physician know of the family history.
