No: Depending on how tight the phimosis, one may need to have surgery.  If the phimosis is not very tight, then conservative management with topical creams and gentle retraction may all that is needed.  If these measures fails, then surgery is the next step is surgery for a circumcision.

Phimosis: Not likely.  Phimosis that is now of "pinhole" diameter usually requires some type of procedure (dorsal slit or circumcision) by a urologist.  Usually forcing the forskin back with such extreme narrowing is either too painful for you or will causes ripping of the skin and bleeding.

Unavoidable: After phimosis surgery some deformity is not unusual and is usually permanent although shouldn't interfere with normal functions. See a urologist if you have further concerns.

Circumcision: Phimosis occurs from inflammation of the foreskin. This can scar it, making it difficult and painful to have erections. Sometimes the skin cracks and bleeds. 
Circumcision can fix this problem.  Under anesthesia, the extra skin is removed, leaving the head exposed at all times.  The stitches are just behind the head, all the way around.  They usually fall out in 2 weeks.  Most men have mild pain.

Evaluation needed: You can live your life as you wish. If this activity gives you pain you should seek an evaluation by a urologist. They can verify what problem exists and explain the options you have to deal with it. Surgery is an option but what is done can vary from a simple release to more involved total circumcision. Do not assume that is your only option.

Old-fashioned way: A guy whose foreskin pulls back all the way when not erect, but is tight during erection, should be able to fix the problem by stretching his foreskin twice a day until it slides back and forth over the glans (head of the penis) easily during an erection. The simplest way to stretch the foreskin is to create an erection and then pull the foreskin back to let the glans stretch the foreskin.

Pearlypenile papules: The tiny white bumps are called pearly penile papules, and are benign, and common in the corona of the glans of the uncircumcised.  Marked tenderness of the glans is very common after circumcision.  Call your surgeon for his/her preferred care. Usually a gauze with vaseline is protective enough, but if swollen, I'd recommend a checkup.

Likely scar tissue: After circumcision for phimosis most likely you developed scar tissue  around corona  ( upper part of the glans , see a surgeon  will release this  'ring 'of scar tissue  by revision  then you will be fine.
