Infective endocardit: Although IV drug users often develop endocarditis on the right side of the heart they may have any valve involved, particularly if there exists a right to left shunt, either from the pulmonary HTN induced by IV drug use emboli or anatomic variances (patent foramen ovale, etc.).  Periodonitis with bacteremia most commonly produces aortic or mitral valve disease, but again any valve may be infected.

The outcome of therapy depends on the diagnosis, identified the cause, treatment, and follow up.

As for now, brush with a mix of 1/3 salt and 2/3 baking soda...it will lessen any edema you have and reduce the pocket depths.

Not really: Periodontal scaling, in a trained hand, is fairly pain free.  With the use of piezo scaler, the gross calculus can be removed easily.  Chelating agent can be used to soften the calculus.  The secret to hand scale is sharp instrument.  Finally,  planning of the root to create a smooth, glass like surface is no longer needed to arrest the disease.  Follow up in 4 wks and detailing is the key!

Periodontal probing: Periodontal probing should be part of your routine dental exam and cleanings. If this is not being done at least once a year you can ask but you should not need to. It is standard care or should be in every dental office.
