When it is soaked: But you should have an evaluation of the heavy period by your gyn for proper diagnosis and treatment so you won't have to deal with heavy bleeding and make sure nothing strange is going on, like an endometrial polyp, fibriod, pre- cancer, cancer , etc. You should go for an exam, blood work and ultrasound and you may need a short 4 minute in offcie hysteroscopy and biopsy to make sure u are ok.

Pad change: You should change your pad when you feel that it's wet.  The amount of blood that a pad contains will vary with the product.  But if you're uncomfortable change it!

Not uncommon: After going a long time with no cycle there is often a buildup of tissue that comes out.  You need to be checked and evaluated as to why these periods are so irregular.

Before now: Its time to see a GYN.  Most women resume cycles within a few weeks of a miscarriage.  You've been without one long enough to be at risk of severe bleeding or prolonged spotting.  You need some blood work and and evaluation by a GYN.  Check a pregnancy test as well as you may have conceived not long after you miscarriage.

See a GYN ASAP : Three pads in one hour is way too much! if you aren't already anemic, you will be soon. I have seen women require blood transfusions because of a "heavy period". Bleeding this much is not normal, and should be evaluated. Your gyn doc may be able to help you in the office, but if you can't get in today, you may need to go to the er. Sometimes the treatment is just a medication,  sometimes a d;c.

Irregular periods and spotting could mean you may have some hormonal imbalance that could be affected by many different factors, including age (if approaching menopause?), nutrition, illness, exercise (or lack of it), body girth, psychological well-being, stress, season, or simple fluctuations of other related hormones (thyroid for example).  U should check a preg test 2.

Rule out pregnancy: Any change in the pattern of your period when there is the chance of becoming pregnant, means pregnancy should be checked for.  If not pregnant, then changes in periods can be a normal reaction to stress, activity changes, recent medication use.  It is pretty common and if you feel fine and are not pregnant, then wait and see.  If it happens again on your next period, it is worth getting checked.

Both: It is more a matter of preference.  Toxic shock syndrome more common with tampons but still very rare especially if tampon is changed regularly...Every 3-4 hours.
