Tarlov cysts: Are an overwhelmingly unimportant finding so don't worry about it. The thyroid nodule is also most likely insignificant but ultrasound may be useful to take a second look so ask your clinician. The great majority of thyroid nodules are not cancerous.

Get further eval.: If you have gotten worse since epidural, you need further evaluation, if not by the physician who did the procedure, by another practitioner.

5 percent: Some degree of peri neural cysts are seen in about 3-5 percent of lumbar MRI scans.  They are often a symptomatic and as such in many cases are considered almost incidental findings.  Meaning a finding seen on an imaging study that does not require treatment or further evaluation.

Yes +/- no: The cysts and the DDD are in a region that could produce sciatic pain, so yes, either of these could be the cause.  But it is hard to know.  What if it is not sciatic pain, for example?  Both cysts and DDD are also found in symptom-free individuals.  Some surgeons use nerve conduction tests to help narrow the source of the problem.  That may also help you. Perineural : Perineural cysts are small fluid containing cysts around nerves.  The exact origin of these cysts are unknown but they are though to be present from birth (congenital).  They rarely cause pain or neurological symptoms unless they are very large.  Most of these cysts are incidental, that is they are found when an MRI scan is done for another reason, and do not need treatment.

The back pain you developed bending over to pick something up was probably from straining your back (spinal disks, facet joints and/or muscles) and not from the perineural cysts.  The cysts were probably there since you were born and just identified since you had an mri.

Most cases of back pain improve without injections or surgery.  It appears your back is improving.

Hopefully this information will be helpful to you.  Since each situation is different i recommend you discus your pain and MRI scan with your neurosurgeon.

Sounds likely: Diseases involving other organs occur such as polycystic kidney or polycystic ovaries occur. The central nervous system is not immune from such malformations.

Back pain: This would be best addressed by a  neurosurgeon as he is able to appreciate the number and location of those cysts and their impact on your back. Otherwise, it would be a pure guess.

No real treatment,: Usually are asymptomatic and do not require any intervention. If exert effect on spinal nerve, then surgical drainage.
