Hard to tell: Perineum is the area between the vulva and anus(in females) or the base of the penis and the anus ( males). I guess somebody has to see it. Is it a tear? Is it after delivery of baby? Is there any skin disorder associated to it? Did you have any injury? Are you riding a bicycle?

Region of the pelvis: Ther perineum is the area between the anus and the scrotum in men and the vulva in women. It generally refers to the surface anatomy,  from the pubic symphysis to the coccyx. Perineal tears and episiotomy can occur in this region during childbirth. The anogenital distance is a measure of the distance between the anus and the base of the penis or vagina & used in the diagnosis of certain disorders.

A perineal tear: is caused by traumatic childbirth: a laceration of the skin and soft tissues which separate the vagina from the anus. They are common, usually superficial and often require no treatment. But more severe tears can result in fecal incontinence/urgency, chronic perineal pain or pain with sex. Your delivering mid-wife or doctor can diagnose the degree of tear and provide repair, if needed.

The way it feels exactly is going to vary slightly between one person and another. Body type, weight, age, muscle strength and how many vaginal deliveries are all factors. Next time you see a gynecologist don’t be embarrassed to ask them to look.

Perineal lesion:   A doctor needs to look but it sounds benign and is most probably a skin tag of some sort. As long as it doesn't hurt, it can be left alone.

Multiple: Menopause and reduced or absent vaginal secretions. A urethral diverticulum. Bartholin's or gaertner's  duct cyst. Bacterial or interstitial cystitis.
