Psoriasis?: Perianal psoriasis, lichen planus and other derm conditions can occur, lead to some of the above. May not be primary fissure, or fungal (what you have been rx'd for based on med list). Unlikely std, but if that is a consideration, then it is a consideration.
See derm, maybe GI or colorectal surgeon afterward.

Liposuction : Liposuction works better on thinner people. Of course, you still need to have enough fat in the areas of concern for liposuction to have an effect. Stubborn, disproportionate areas of fat in someone close to their ideal weight are ideal for the procedure. There is lower risk of gaining weight after liposuction which can negatively affect your results.

Liposuction has the ability to selectively remove fat deposits exactly from the areas where we want it removed. This is different than weight loss, where the fat goes away following your genetic road map.

It : It depends on whether the surgeon, usually a board certified plastic surgeon--is planning to do a very small area, i.e. On the chin.. Or is planning to do something as radical as complete body re-sculpturing. In the first instance, it is likely that local anesthesia might be the anesthetic of choice. In the second instance, because of the large amount of fluid and local anesthetic that is frequently injected during these lengthy procedures, it is usually highly recommended that the patient undergo a general anesthetic. It is absolutely essential to investigate where your procedure will be done and to make sure that the facility has a board certified anesthesiologist performing your anesthetic (unless the surgeon is injecting local anesthetics without sedation). Likewise, it is also important the all of the proper equipment for resuscitation be available in the facility.

Probably: Hi. I see you're a male. You probably had liposuction of your love handles rather than of your hips. The volume aspirated could be fine, depending on the amount of fat deposits you had before your surgery. Also, 3 weeks is still early to see final results. Your best option is to talk to your surgeon about this and maybe go over your before and after pictures for a better idea of your results

Liposuction: Yes, a person can have repeat liposuction in the same area. It is much better to remove too little fat from an area than too much. It is always possible to take more out, but difficult to correct deformities from having too much removed.

Possibly: If there's still some pockets of residual fat, then secondary liposuction (or perhaps coolsculpting) could be used to further shape your contours.  However, if it's really just some zones of loose skin, then lipo would not help with that.  Best to see your local plastic surgeon to discuss the options.

Are you nuts?: Your body is going to change a lot during and after pregnancy. If you really have to have liposuction, wait until at least 6 months after the baby is born.  I suppose liposuction could be performed during pregnancy, but there would be risk involved due to the anesthetics used. I doubt you could find a reputable surgeon willing to do it.
