i have had a great deal of tension in my pelvic floor for years. my pelvic exams and pap smears are normal. what might cause this ongoing discomfort?

Muscle imbalance: Not uncommon:pelvic floor muscle imbalance; +/- paradoxical spasm/contraction when relaxation should occur, vice versa. Can be rel. Extl (vaginismus), or intl. Can be assoc w/ dropped bladder and/or rectum commonly associated with pregnancy, pelvic surgery, e.g. Hysterectomy, internal hernias, ; some can be assoc with history of abuse. May be related to bowel issues. Physical and radiol/sono tests.

Pelvic floor damage: The most common cause of pelvic floor damage is a vaginal delivery. Common symptoms include urinary incontinence, fecal incontinence, and uterine or vaginal prolapse (something hanging out). There are pelvic exercises that can help strengthen pelvic floor muscles and a physical therapist can be helpful.

Continuum: There are numerous disorders that fall within a wide spectrum. They can be independent or closely related. 

There are numerous tests as well as your history that would be used to differentiate the two.

: Your question is unclear. If you still can have an erection your penis is fine. Self diagnosis of 'prostate symptoms' is often erroneous. So pl state tour symptoms. In the meantime try soaking the pelvic area in hot water three times a day.

Pelvic floor. : You wouldn’t feel like you have a urinary tract infection simply from pelvic floor abnormality. However, the pelvic floor abnormality can affect your bladder in a way that makes you more prone to urinary tract infections.

for ~week ive been leaking urine, often soon after i sit but only after standing a while. have pelvic floor dysfunction, but no recent issues and no uti symptoms. no coughing/sneezing, etc but my urine does smell a little off. any ideas as to cause?

: With history of pelvic floor dysfunction and urine leak with foul odor I think you may experience pelvic organ prolapse such as cystocele or uterine prolapse.

Yes: Heavy weight lifting that increase abdominal pressure can push tissue down further but its not as likely as you'd think. Higher risk factors such as multiple kids, chronic coughing are more significant. 
Your vagina wont become loose if you exercise and lift weights in moderation.

Many: Pelvic floor disorders covers a spectrum of pelvic organ prolapse which is when the pelvic organs are falling down from the normal postion. The bladder, urethra, rectum, vagina and uterus can all be involved to a minimal degree or extensively involved with complete prolapse, which is where the uterus or vagina falls outside completely. Incontinence is also commonly present.

how does a mans pelvic floor muscles get overtight or overused?

Accepted Answer

Almost impossible: Men have difficulty even knowing how to exercise the pelvic floor muscles, so an overuse injury seems to me to be impossible.  If you think you have this, it is probably something else.  
And no, a lot of sex wouldn't cause the pelvic floor muscles to be tight or overused.  Generally, sex is a very good thing for men.  But if you are having symptoms down there, best to get checked.  Good luck!
