CAP: You may be confusing "pathophysiology" with the defining features of cap.  This is pneumonia in a person who has not been in contact with medical facilities in contrast to pneumonias which occur in hospital, nursing homes, etc.  They generally occur by aspiration of microorganisms from the mouth or nasopharynx into the lung, often in people with prior viral illnesses or other predisposing factors.

Low oxygen?: One possibility is having low oxygen. A disease such as pneumonia fills the alveoli with infectious/inflammatory debris which prevents gas exchange. Maintaining an appropriate oxygen level in the blood becomes more difficult, especially with exertion. A bacterial infection also causes lots of inflammatory cytokines which can also make you feel unwell.

Two diff diseases: These are two different diseases. Pneumonia is an infection in the lung that causes fluid to collect within the air spaces (maybe w/in a lobe or more diffuse). Rheumatic heart disease is caused by the body attacking the valves of the heart in response to an untreated/inadequately treated  streptococcal (most often group a) infection ("strep throat" or other).

CAP is…: An infection of the lung(s) in a person not recently hospitalized/in a nursing home/in a rehab facility. Microbes involved in cap tend to be less virulent than those acquired in hospitals. Most common is streptococcus; atypicals (walking pneumonia) include legionella, mycoplasma, & chlamydophila. Viruses common in infants & kids. Pneumocystis seen in immunocompromised.

Recurrent Pneumonia: One common cause is misdiagnosis.  Not knowing your case, i can't say this is what's going on, but a lot of disease get called "pneumonia" when in fact there is something else going on.  There are some cause of recurring pneumonia however such as immunological system deficiencies, aspiration (choking bacteria into the lungs), and underlying lung diseases that haven't been diagnosed.
