Without, the high calcium will prompt imaging studies in search of neck or nearby mass.  This will be excised.  1 in 1000 people needs a parathyroid tumor removed but the large majority are benign and many parathyroid cancers are cured by the surger.

Patients with parathyroid carcinomas are more likely to have symptoms, a neck mass, bone and kidney disease, increase calcium in the blood, and very high serum parathyroid hormone.Treatment is surgery. It is resistant to chemo or radiation.

Majority of primary hyperparathyroidism is detected in asymptomatic patients on routine laboratory screening panels.  The parathyroid glands are quite small even when enlarged.  It is not easy and very difficult to feel a parathyroid tumor.
