Likely: Hyperventilating during a panic attack can cause those symptoms by blowing off too much carbon dioxide. Breathing into a paper bag can reverse this carbon dioxide loss. I strongly recommend stress management therapy or cognitive behavioral therapy going forward. Medication may be temporarily prescribed as well.  Ask your physician for a referral.

often, my hands lightly shake and i don't feel good. could it be an anxiety/panic attack or something else? i haven't been diagnosed anxiety.

See doctor: You're 16 and likely healthy. But first get checked by your pcp to make sure there is no medical reason for your hands shaking often and "not feeling good." Write down when this started, any triggers, what helps, any medications prescribed or OTC you take, any alcohol or drug use, caffeine intake, cigarette smoking and review with your doctor. Ask if anxiety could be the cause. Hope this helps.

Yes: Red hands and feet are your body's response to the stress of a panic attack; however, panic attacks are not normal and should be treated.  There effective treatments for panic attacks, start with your doctor to determine if medications would be appropriate and seek a psychotherapist to work on regular coping strategies for keeping stress at bay and specific relaxation techniques to prevent attacks.

i felt weight in my chest and stomach then my hands and feet were cold. i controlled to feel scared bc it may lead to my panic attacks, why is that?

Anxiety: Feeling weight in the stomach and chest, and hands and feet feeling cold are likely symptoms of anxiety. Your fear that it may lead to panic attacks seems to indicate feeling overwhelmed with stresses of relationship, unresolved conflicts, school, and peer pressure at age 16.
Alcohol and drug use can make these symptoms worse.

Lo dose/cbt: Dose is too low speak with your doctor - also ssris for long haul and 
cbt-cognitive behavioral therapy with or instead of the meds.

Get evaluated: Panic attacks can mimic other conditions or be in and of themselves prohibitive of being able to function.  Typically, making sure you are safe and not going to be a hazard to yourself or the public is a prime concern.  It is usually advised to get to a proper medical facility the safest way possible to be evaluated.

Panic attacks: Part of the definition of panic attacks includes the mental anxiety/panic. Some attacks are thought to be triggered by physiologic stresses. It may be possible that the spectrum could include a form without conscious panic....But i can't find it in the literature. Whether some modification in the presentation can be caused by the drugs?

Possible: Not associated with panic reaction, but could be migraine aura, especially if followed by headache.
But other possibilities exist, such a pinched nerve in arm or neck, or even brain circulatory issues.  See your doctor and get examined.
