Unlikely need tests: Ovarian cancer initial stages will not have any pain a reason for delayed diagnosis , you could have anything from endometriosis, adhesions , pelvic infections hormonal imbalance etc, you need to see your gyn doctor for tests and treatment.

Spermicide?: No STD can start to cause painful urination or any other symptoms sooner than 2-3 days after exposure. Perhaps the condom contained a spermicide, and some spermicides (e.g. nonoxynol-9) can irritate the urethra, with painful urination that clears up within a day. No infection or inflammation intereferes with erection, maybe anxiety or worry about the event. But see a doctor if symptoms continue.

: It is difficult to make a correlation between bowel movement and painful urination (dysuria). So do you have pain with voiding when sitting on the toilet or just only during bowel movement? Is voiding completely normal (good stream, no pain) on standing?
Are your bowel movement normal? 
Whatever, a urinalysis needs to be done first for ruling out bladder and prostate issues.

UTI: Have you had a positive urine culture,colony count and sensitivity to document the presence of a urinary tract infection and use of an effective antibiotic as shown by the testing? If no definite UTI, your symptoms could be due to urethritis caused by intercourse with inadequate lubrication. In addition to foreplay, use of KY jelly lubrication,&amp; or intercourse on your side or you on top would help

See gynecologist: It seems that your uterus is getting sensitive as it changes shape and thickens prior to your period.  It also makes sense that the bleeding is heavy with this.  You should see your gynecologist for evaluation of this as it is very likely treatable.

Many things: A number of things can cause painful urination.  A bladder infection, interstitial cystitis, a vaginal infection, or atrophic vaginitis can all cause painful urination.  An evaluation typically begins with a history of the problem, a physical exam and a simple urinalysis.

UTI: Painful urination is most often due to a bladder infection.  Sometimes a urethral infection from a sexually transmitted disease is also a cause.  Start with a urine test, and if it is consistent with a UTI then antibiotics will resolve it.  If you have any vaginal discharge then you should also have tests for stds.

Urination: The most common cause if a urinary tract infection.  Often this requires antibiotics.  There are other etiologies however.  You should get a urine culture done to see if there is bacteria and if so, will need antibiotics.  If no bacteria, further work up is needed.
