Lump: Most common reason for painful lump on testicle would be an epididimytis. Other painful lumps would be vericocele or hydrocele. Painful lump that is worsening should be checked out. See if you can be seen by your provider.

Being on or in scrotal wall, it can be easily palpated by ordinary hands; if inside the sac, a tiny lump is not easy for ordinary inexperienced hand to differentiate if it's on or inside or outside the testis, but such indeed terrifies some men. There's a small structure up there called the appendix testis (yes, appendix testis) that can get clogged, which doesn't mean much but should be verified just to make sure it isn't something more serious, especially at your age. It hasn't been emphasized enough that men in their teens and twenties check their testes for masses. I'd suggest getting your girlfriend to do it...Fun

No lump growth or change in many, many years makes a lump likely non-cancerous... but new lumps are unknowns and need to be examined closely.

If there is a cancer, it is treatable by removal of the testicle, and getting follow-up care.
