They can be.: A common misconception is that if a woman has a tender, painful lump in their breast, it is unlikely to be cancer.  While it is true that many women with breast cancer do not have breast pain, the presence of pain does not exclude cancer as the cause.  Bottom line:  a new lump in the breast, painful or not, warrants medical evaluation.

No: Breast tissue are respond to estrogen mainly. Having high testosterone should not do that. Check your prolactin as well. High prolactin can cause breast tenderness.

See your Dr.: I am assuming you have a primary care physician or gyn serving that purpose.  The first step is to go and be examined.  Most breast lumps are benign so stay calm but don't put off this examination.  Your doctor will probably recommend some form of imaging and may recommend that you see a breast surgeon.   I think that would be a good idea and my recommendations for first steps.

See a doctor: Needs medical evaluation and mammogram with possible breast ultrasound. Painful breast cysts will often resolve after your menstrual cycle or can have needle aspiration in the office. But, needs an exam to be sure it is benign.

Many things: This could be many things. It could be an infection or abscess. It could be a fibroadenoma, which is a benign tumor of the breast. It could also be just very fibrocystic breast tissue. Abscess or cyst: There could be an abscess , pus collection, or a  cyst or  fluid collection or mastitis or an infection of your breast gland get this checked out with your primary file need an ultrasound to determine if you need something to be drained or biopsied. Do not ignore this. You may also need antibiotics. Good luck.

Infection: You could be experiencing mastitis, which is an infection due to clogging of the milk duct.  You should alert your gyn and get a complete breast exam.  You may need an ultrasound.  Do not delay in contacting your doctors.

Puberty: Pubertal gynecomastia may effect the majority of males as the begin their pubertal transition. Most are minimal and last 6m to a year, but occasional longer episodes do occur (1-3yr) spongy glandular tissue directly under the nipple defines the process.

PCP: An urgent care center is better-designed to treat emergncies;  most pcps have more experience dealing with breast disease.  However, if you think you have a breast abscess (redness, swelling, fever, severe pain) and your pcp is unavailable, it is reasonable to seek out immediate attention at an urgent care center.
