Without : Without an xray and examination, it is very difficult to know for sure what you have done to your knee/leg.  The most common possibilities include a fracture, ligament sprains or tears, cartilage tears, and a deep contusion/bruise to the bone/joint.  You really should be evaluated as soon as possible by an orthopedic surgeon, or perhaps at least in a minor emergency room.  In an er, xrays can be taken to evaluate for a broken bone.  If it is more of a soft tissue injury, and if the diagnosis remains unclear after a physical exam by the physician, an MRI may be needed for additional information.

Knee pain: You may have a problem with the knee ligaments, muscles around it, menisci, bones. More history is necessary as well as physical exam.

Hoffa pad NOT: The Hoffa pas is below the knee cap. Your problem sounds like possibly fluid in the knee due to a meniscal or cartilage injury. You should be evaluated to ascertain the problem. In the meantime you should apply ice to the area after activity. You may try capsaicin cream( a small amount) at night for relief. Oral anti-inflammatories should help if you have no history of a stomach ulcer renal diseas

Twisting motion: followed by injury to the knee likely points to meniscal injuries.   Though it is hard to really damage the cartilage in your knee (meniscus) with such a non-weight bearing motion while supine, unless you have had prior injury and only exacerbated it.  You should see an experienced healthcare provider who can examine and diagnose you appropriately.

i was walking the other day on the sidewalk and i stepped down but the sidewalk was uneven and felt immediate pain in the medial side of the knee. could i have possibly torn my meniscus? the pain is very intense hurts to straighten or bend. help?

: A meniscus injury is possible, but other problems may have occurred.  You need answers and appropriate therapeutic intervention.  See an orthopedic surgeon ASAP

This is not normal: and may be a number of different issues. Make an appointment to be seen by your health care provider tomorrow if possible. In the mean time ice the joint liberally.

Clinic: If you have no insurance, call the closest medical school clinic. Usually listed under a university name.  Ask receptionist for name of the walk-in clinic or their general medical clinic. Then call the clinic and explain you are uninsured. Most likely they have a volunteer no cost clinic. You have arthritis or knee injury and need to be seen. If all fails, go to er -- they cannot turn you away.
