Quite possibly: gastro-laryngo reflux.  Try 2 OTC famotidine daily for two weeks.  If no improvement see your PCP again and inform him or her as to what you tried.

shoulder blade hurt from working out, can feel on/off bouts of pain in right chest sometimes. comes and goes after few seconds. what to do for this?

Shoulder pain: You need a diagnosis from your doctor so that the proper treatment can be given.  It could be anything from muscular pain to cardiac symptoms.  After you know what is happening you can adjust your workout routine accordingly.  Peace and good health.

: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water. Wish you good health!

Needs evaluation: This could be something simple like reflux or something as complex as a gastric cancer or hiatal hernia needing repair.  If you have not had an egd (upper scope) then you may need one, please see a doctor because over a year is too long to doctor yourself!

Maybe not : The liver is under the lower right portion of your rib cage, so you may feel pain from your liver in the right lower chest. One cause would be hepatitis or even gallbladder pain. See you doctor to find out more.

Location: Liver pain usually be in the right upper abdomen or sometimes referred to the right shoulder blade if the right diaphragm is being irritated.  Right chest pain usually is in the right chest.

There are several possibilities on your diagnostic list, cholecystitis, fatty liver, pleurisy, pneumonia, acute cardiac syndrome, pericarditis, and more.....

i have a sharp pain on right chest and back. i feel it when i lie down and sometimes when i breathe deeply, what could be the cause?

R sharp chest pain  : Could be cardiac -unless you smoke are on BCP or  have family history of clots where may be clot  ,  but more likely reflux (lying down)- try ranitidine antacids) vs gallbladder , if you have cough may be  pneumonia. Would f/u/ w/ doc today
