Concerning: We get concerned about abdominal pain and constipation and this needs to be evaluated. A full physical exam is needed. Tests needed include a colonoscopy and Ct of the abdomen. Please get evaluated. Good luck

Neuropathy: This incomplete emptying of rectal contents is seen with neurogenic disturbance which may be a complication of your type 1 diabetes. Using rectal glycerin or dulcolax suppositories may help evacuate better. Fleets enemas can be used if you need more help emptying rectal contents. A gastroenterologist or a proctologist may be helpful for you.

Genetic?: Part of this may be genetic (your mother's body shape can tell you a lot).  Have you tried weight training and abdominal exercises to tighten your muscles?  Tobacco smoking and alcohol intake also direct the fat to your abdomen.   Consider consulting a trainer at your gym.

Constipation: This sounds like constipation. I would start Metamucil one tablespoon night and morning, increase your ruffage input and increase your fluid intake. You may also try a glycerin suppository to ease your anal discomfort

Emergency Department: An 18 yo woman with progressively worse, sharp right lower abdominal pain, associated with vomiting, over a few hours.

Patient may have acute appendicitis or other urgent/emergent medical condition. Urge patient to go to nearest Emergency Department promptly.

Some ideas: Without an actual examination, I cannot give specific medical advice on your particular health,
the colon and intestines are there, colitis, irritable bowel, even appendicitis
The urinary tract is there. Kidney stones, infection, sometimes other bladder issues
Muscles are there and minor strains can occur.
Nerves go there. Rarely shingles can occur there. More rare (and that's RARE!) other nerve.

Appendix vs. Muscle: Right lower abdominal pain always raises the issue of appendicitis. Usually it is tender and is accompanied by loss of appetite. But these are not hard and fast rules. When in doubt have a doctor put a hand on your stomach. You may have to come back at short intervals to be sure. It could be muscle strain, but better safe than sorry.

Exam needed : Although your symptoms could be from an ovarian cause, acute appendicitis is by far the commonest cause of right lower quadrant abdominal discomfort especially if there's associated tenderness. If your symptoms persist, I recommend going to the hospital emergency room where prompt evaluation and testing are available.
