when i laid down in bed last night after an hour or so my right ankle and calf started to throb sharp intense pain . i woke up this morning no pain but shortly started same pain in left ankle . very intense . nothing is helping stop it . please help?

: Please see your physician for an examination, D Dimer test and Doppler ultrasound to rule out a DVT as indicated. It may be only a muscle cramp, stay well hydrated. Watch for signs of shortness of breath. If this occurs, see the Emergency room.

Have it evaluated: You need to determine the cause of the pain first. Have an orthopedist evaluate the ankle.

You Got it.: It sounds like you have a good idea of just what the contributing factors are. If you activities are going to be limited and less time on hard surfaces are in the future, then the problem should resolve. But, if the activity is more or less of a more permanent nature, you will have to get a great pair of shoes. I have found that KEEN UTILITY oxfords are just what the doctor ordered.

Sprain?: First thought is that you had a sprain injury. Does that make sense? If so, then local ice with some compression (like ACE bandage) would help, along with rest and elevation. Hope that helps. If not, please get back with more information.

i have had psoriasis x 6 months, i have had pain in left ankle x 3 days, no history of injury. should i be concerned about psoratic arthritis?

Probably not: Psoriatic arthritis typically affects the joints of the fingers and toes only, larger joints like the ankle are very rarely affected.  As it is, only a small percentage (~10%) of patients with psoriasis develop psoriatic arthritis.  I would look for other causes of your ankle pain.

Unlikely: Unless MRI showed spurs, it is most likely instability.

Torn ligament: He may have ankle instability due to a torn ligament in his ankle. Can be examined by doctor and MRI may help. Can also try wearing an ankle brace.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor. Depending on your age, weight, history of trauma, it may be arthritis. For the short term you may take acetaminophen, not more than 4 g/day and no alcohol. 
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.
