Hemoptysis: That is the medical term for coughing up blood. The pain in your arm is of no concern. But you must see a doctor about your hemoptysis. You will need a chest X-ray and possibly a CAT scan of your chest.

Tetanus shot: Although the tetanus vaccine does cause more muscle irritation and discomfort than other vaccines, it is unusual for you you to still have pain in your arm.  In most cases, the soreness resolves in 5-7 days at the latest.  You definitely should see your doctor about your symptoms.

i am experiencing  cough (quality: blood in phlegm, with phelgm or mucus)  and arm pain. the following also describes me: chest congestion. green and yellow phlegm with blood..  was prescribed zpak. but afraid to take. is there a substitute?

Take as prescribed: This sounds like a potentially serious infection, maybe even pneumonia. "Afraid to take" azithromycin (Zpak) is not rational; it is a very safe antibiotic with few side effects. Not treating your infection is MUCH riskier than any potential problem from the antibiotic. Take it as prescribed; if you remain concerned, discuss with the doctor who prescribed it.

Herniated disc: Coughing increases pressure in the spine and if there is an underlying  compression of a nerve or the spinal cord itself, pain can be produced.  Best to be examined and perhaps have an MRI of the neck perfored.

Possibly: Severe coughing could conceivably lead to a cervical disc herniation which could then cause bilateral arm pain.  This can be diagnosed on mri, so see your physician if symptoms persist.

Serious: Seek medical attention right away.  Any weight loss? Poor appetite, dark stools. Fatigue?.  Get a chest imaging study and an upper endoscopy.  Have a test for blood in your stool.
