If you pay attention to your senses (direct representatives of 'reality'), it's harder to dissociate.

Stay active: Exercise is a great way to relieve stress. Your body releases endorphins during exercise, which can help you feel calm. If you exercise 30 - 60 minutes a day, your stress levels can improve. When stressed, take 5 slow, deep breaths with your eyes closed, then roll your shoulders forward 5 times, then back 5 times. This will slow your heart rate and release tension in your neck and shoulders.

Psychotherapy: A good Health Psychologist can really help with this problem - you need to trust them and be willing to learn and it will be successful. Best wishes. Avoid medication unless they think it is really needed - that can start you on a long road and if you can do it without - you'll be better off.

Rule out Vision : Before looking for psych or neurological reasons, you need to rule out that there are no actual vision problems.  Visual acuity issues can cause these symptoms.  So can sensitivity to fluorescent lighting.  If your eyesight is 20/20, try reading in natural light or incandescent.  If the sx go away, use this as a guide for your reading activities.  If not, an assessment by a neuropsychologist is in.
