i am a 43 yo non smoker with an ovarian cyst. which pill is best a pop or estrogen/progestin? or are there other options?thank you.

All are fine: Ovarian cysts are common and while they are more frequently associated with progestin only pills, if you are healthy any of these options ( including a nuvaring, nexplanon, iud, as well) are fine. The best birth control option is the one that you feel comfortable with, tolerate, and you will use correctly. Good luck.

Combined OCP: There is not a great answer to this question. Many types of ocp's can be helpful in preventing large cysts from forming. However, this is not a fool proof treatment either. Many people will still develop cysts. I would recommend trying any combined tri or mono-phasic pill either to a cyclic fashion (monthly or quarterly periods) or in a continuous fashion (skipping periods) to see what works for u.

Ovarian cyst: If you have a large ovarian cyst this needs to be evaluated by an Obgyn to determine if needs to be removed or if it is likely to resolve on its own.  The term cervical cyst is unclear in meaning. An Obgyn should evaluate this to determine if further work up is needed. Neither of these would lead one to suspect a hormonal imbalance.

day-3 ultrasound shown a 6.4cm by 4.3cm ovarian cyst. doctor said my estrogen level is 106 and so i can start clomid (clomiphene) 75mg. good to start? or wait?

I would not: start a patient of mine under these conditions, but your doctor may have  different approach with different reasoning. You should ask your doctor to clarify if you are uncomfortable with the decision to start.

can you have normal testosterone and estrogen levels with multiple ovarian cyst and still be considered to have pcos?

Accepted Answer

Yes: Pcos is a clinical syndrome.  It is not diagnosed by blood test results, though high male hormone levels, or other tests suggestive of Insulin resistance, can support the diagnosis.  If you have problems with irregular periods, weight gain, excess hair growth, you can be diagnosed with pcos, despite normal lab results.

i had an ovarian cyst rupture 7 days ago the symptoms don't seem to be getting better. the sharp pain on my right sides comes and goes but i have been severely bloated since the cyst ruptured. my obgyn started me on low dose estrogen birth control. ?

Pelvic pain: Pelvic pain is complex as there are several organs in the pelvis. The ovaries, fallopian tubes, uterus, cervix, bladder, appendix, intestines, colon and rectum can all be the source. Pelvic pain requires and examination to help organize the evaluation. Right sided pain could be for concerning for appendicitis

Ovarian cysts: At 42, the ovaries require more stimulation to achieve a release of an egg (ovulation).  Sometimes the egg does not rupture the follicle (small fluid filled area around the egg before ovulation) and a persistent cyst results. At "4 cm." it will eventually rupture and pain will result temporarily.  With PCOS you may be better taking low dose oral contraceptives.  Ask your doctor to consider this.

Watch: These are relatively small and can be reasonably followed with serial sonograms.  You can also do a blood test (ca125) to help make following these conservatively safe.

Patience vs. Surgery: Most ovarian cysts in young women are "functional cysts", which evolve from a follicle that you ovulate from, then grow and finally resolve spontaneously over several weeks.  Birth control pills do not speed up the resolution, but do decrease recurrences.
If the cyst persists then surgery, often laparoscopic, is necessary to remove the cyst and have it tested for cancer.
