Work up includes blood tests and supplements with calcium with vitamin D. Left untreated, this will lead to fractures n affected bones, so do get evaluated for parathyroid, diet choices, vit D, calcium, activity review. Endocrinologists specialize in this, but discuss w your primary doc

my doctor prescribed  me fosamax (alendronate) for osteopenia is it really necessary to take this medication and what other treatment can i do instead?

Strontium, Vit D3: Fosamax (alendronate) and related drugs for osteoporosis may do more harm than good.  I recommend a broad spectrum multi-mineral supplement, high-dose vit d3 (5-10, 000 iu per day), plus strontium (as carbonate or citrate) available from a health food store, in doses of 600 mg per day.  Strontium has been demonstrated to significantly increase bone density and strength.  Also, weight training will help.

Based on the provided information your current regimen is effective and medication is an option but not required. To guide you, if possible get a FRAX calculation for a 10 year risk of hip and other major fractures.

Exercise and vit D: Weight-bearing exercises will help strengthen your bones. Calcium and vitamin d supplements also help-i recommend a blood test for vitamin d (25 hydroxy vitamin d), it should be 30 or higher. Some people need much more vitamin d than what is recommended to get their level up to 30. Be active: Start with low impact exervcises, such as tai chi or similar activities.

Having a diet that is well balanced and includes enough calcium is quite important as well.

Try to avoid diet things that cause you to leech out calcium from your bones, such as caffeine.

Yes: Likely, more so for osteopenia than osteoporosis. Weight bearing exercises, making sure vitamin d and calcium intake is maximized. Vitamin k2 is also good and under appreciated in the medical community.

No: increases risk: Osteopenia means your bones have lost some of their mineral content, but probably not enough to greatly increase fracture risk. Osteoporosis is more severe mineral loss that does increase the likelihood of fractures. Neither causes pain unless you actually break a bone. (if you have either, be sure your vitamin d level is high enough. Your doctor can do a blood test. But don't overdo vitamin d.).
