sometime i feel a big boom in my chest followed by a pause of 1-2sec then my heart beat fast and go back normal. what can cause this if heart tests ok?

PVC: The heart has a pacemaker (PM) or internal clock in the upper chambers (atria) that sends a signal to the lower  chambers (ventricles) which is delayed so that the ventricles contract after the atria. Premature ventricular contractions (PVCs) can occur spontaneously or be triggered by adrenaline or drugs and are always followed by a compensatory pause to return the heart to its previous rhythm.

what causes an e-regular heart beat? i have just found out i have one.

Many things can: An irregular heartbeat can have many causes from very benign ones to serious ones. It is important to review with your provider to be sure you are asymptomatic. Often an ekg is needed to be able to say more clearly what the irregularity is which can help decide its cause. Sme causes are from the heart itself, some can be meds, electrolyte issues, blood flow, caffeine/alcohol.

No: Varying heartbeats could mean that you are having an arrhythmia  (ie that all the heartbeats are not being generated by the usual pacemaker in your heart).   Sometimes this can happen in a normal heart, but it can also mean some kind of heart disease (such as coronary artery disease) or that something is stimulating your heart (such as drugs, alcohol, an overactive thyroid, etc).  See your md!

i am suffering from shortness of breathing for last one year. sometimes it increase. sometimes it is all okay. sometimes heart beat faster.

Shortness: Of breath can have many causes...Too many to list here. Sounds like you need to have this evaluated. See your doctor for further workup.

Extrasystoles: Your description suggests extrasystoles, that are common in normal individuals. Sometimes due to excess caffeine or stress, most of the time remain unexplained. Any associated symptoms, such as lightheadedness, chest pain, nausea among other should trigger workup. Hope it helps.

Depends.: If you are six weeks and one day by your dates and the US didn't show a fetus you may just be to early to see the baby.  If you can see and measure a fetus but cannot see a heartbeat then that is abnormal.  Hope this helps.

PVC: If t is an infrequent, brief event with just one abnormal beat it is probably a pvc.  Premature ventricular contraction means the ventricle part of the heart delivers a beat before the normal pacemaker.  It happens very commonly.   Low potassium and other electrolyte problems would make this more common.  The cause could be significant but the PVC itself doesn't seem to cause problems.

Premature beats: Usually not a problem depending & type & amount.  Recommend further evaluation that will likely include holter monitor & echocardiogram ...Talk to your physician.

Spasm: spasm in one of the extra ocular muscles
