Exam needed: Vit D deficiency may be associated with a general increase in pain perception, but you described a very isolated pain syndrome which requires investigation. Call your FP, PMR, Neurologist for an exam to determine where the problem stems and get non invasive testing (EMG, NCS, MRI) for an accurate diagnosis. Good luck

See a doctor: Any symptoms that involve a severe neurological problem need a doctor to examine that person and also get a brain and or spinal MRI as part of the evaluation. If you can get that kind of care, do it immediately !

Evaluate Neck: If you are not improving have your doctor order a cervical spine mri.  Many times a herniated disc in the neck can cause arm pain especially if you have not improved with the medications and physical therapy.  If it is a herniated disc you may need evaluation from a spine specialist to determine what other options might be needed (injections, surgery, etc) to help reduce pain.

Your : Your history sounds very complicated.  Forty surgeries is amazingly extensive.  Please see your surgeon immediately as your symptoms are very concerning.  If you have weakness, fever, or vomiting then you must be seen immediately.

Atypical migraine : You probably are suffering from migraine and need proper treatment for you should visit and urgent care Center or go to or visit ER. Meanwhile use excedrin migraine or ibuprofen 400mg tablets every 6 hours. Drink a lot of water. Caffeine sometimes help in moderation
