Carpal Boss: The surgeon makes a 2-3 CM (1 inch) cut across the area of swelling. The abnormality usually in the area of the base of the index and long finger  arthritic part of the joint and the swelling over it is cut out. The skin is then closed. Then dressing is applied.  This is typically an outpatient surgery.

Well, remember what: I recommended....1st you need to verify the Dx of CTS you say you have in both hands.  Have you seen a neurologist, gotten an EMG &amp;/or NCV study?  If not, I would consider that mandatory for you to avoid unnecessary surgery.  Next, conservative treatment should always be #1, #2, #3 things to consider before surgery.  We like to expedite surgery based upon EMG results, loss of hand strength &amp; pain.

Median nerve release: Carpal tunnel surgery can be done endoscopically (through a camera inserted in a small incision) or open (standard surgery exposing the internal anatomy).  Basically, it is a median nerve release but cutting the carpal ligament which holds down the nerve.  The median nerve is one of the 3 major nerves running through the wrist.  When this nerve is compressed, it causes carpal tunnel syndrome.
