Other way around: The hiatal hernia does not cause a large abdomen, rather the other way around - obesity and overweight are risk factors for developing a hiatal hernia.  If you have a hiatal hernia, you should see your physician about a weight reduction plan to see if this will help you reduce symptoms.  In some cases where bariatric surgery is being done, the hiatal hernia can be fixed at the same time.

: If this is a sliding hiatal hernia symptoms could be heartburn and/ or regurgitation,worse on reclining or bearing down or feeling full soon after meals. If part of the hernia is stuck in the chest, chest pains,vomiting,and sense of difficulty swallowing can occur.

No: Depending upon what has already been done, one would think an egd (scope down the throat) would be in order and pending findings, perhaps more testing (ct or other imaging).  You may need to see a gastroenterologist and or a surgeon dealing with hiatal hernia.  The weight loss is very concerning and it appears more is in order for you diagnostic wise and treatment wise.  Get more evaluation.  Luck.

: The numbers here describe the location of your hiatal hernia.  Using the gastroscope as a sort of tape measure and using your incisor teeth as the starting point, the hernia pushes the z-line up to 38cm. The narrowing is more concerning if it narrows so much that the hernia becomes “incarcerated” and stops sliding. There is no number here to define the width of the hernia.

please talk to me about melatonin vs ppi's for gerd...i have a moderately sized hiatal hernia.

Go with ppi: I know of no role for melatonin as treatment for reflux made worse by hiatal hernia. Melatonin is a sleep aid. Ppis are proton pump inhibitors and reduce acid production in stomach at the source - helping to relieve reflux issues common with hiatal hernia.

Give it more time: One of the causes of early hiatal hernia repair failure is sudden increase in pressure inside the abdomen like coughing, vomiting, lifting heavy stuff etc 
Body takes 6 weeks roughly to lay down the matrix needed for healing, tissue continues to heal for months afterwards but at 6 weeks after surgery it has reached 80% strength. Running jogging should be ok at this stage .
